The new form is now two pages and both pages must be completed to be valid. Please contact their staff with any questions concerning the new form."
- Per the Commission on Postsecondary Education under Fingerprinting Update.
The current 2-page form (Form 40c) required for fingerprinting services can be found on the website of the Comission on Postsecondary Education. Please make certain all fields are filled correctly; if you have any questions or concerns over the paperwork, our fingerprint technicians can help you through the process.
