Most of the divisions within the state of Nevada that require the process of fingerprinting may require additional paperwork to be provided in addition to the fingerprinting process. Some licensing boards may require a certain form that is to be signed by the fingerprinting vendor. Others may simply request the fingerprinting process itself. With so many divisions and agencies within the state of Nevada, it can become quite confusing to most on the exact process in which your division or licensing board is requiring.

Below is a small list of general information as well as necessary forms we have collected from various divisions and licensing boards within the state of Nevada.
Nevada Commission on Postsecondary Education
"Effective June 2, 2020, the Background form 40C has changed due to federal requirements, CPE will no longer be permitted to accept any earlier forms for processing. The new form is now two pages and both pages must be completed to be valid. Please contact their staff with any questions concerning the new form."

The current 2-page form (Form 40c) required for fingerprinting services can be found on the website of the Comission on Postsecondary Education. Please make certain all fields are filled correctly; if you have any questions or concerns over the paperwork, our fingerprint technicians can help you through the process.
The updated Form 40c requires a Signature and Stamp from CPE Staff; we will be unable to take your fingerprints unless this document has an official CPE staff signature and stamp.

Nevada Division of Insurance
The Nevada Division of Insurance requires a Fingerprint Receipt Form as well as a Waiver . B&D Fingerprinting Services LLC can provide you with a Fingerprint Receipt Form which will provide the division with the date of processing and a transaction number. Nevada Gaming Control Board
The current form (Form 28) required for fingerprinting services must be provided by your employer's Human Resources and brought in with you in order to process your fingerprints. This form is normally completed online, allowing you to print the form out at your leisure.
Please make certain all fields are filled correctly; if you have any questions or concerns over the paperwork, you may check with your Human Resources Department. Our fingerprint technicians can also help you through the process.

Nevada State Board of Massage Therapy
The fingerprinting process for Massage Therapy covers new licensing and renewals. B&D Fingerprinting can process your fingerprints and submit them electronically to the Nevada State Department of Public Safety where the results will be sent back to the proper division. The current form (Fingerprint Voucher) required for fingerprinting services must be provided by the Board of Massage Therapy and brought in with you in order to submit your fingerprints. Nevada State Board of Nursing
The fingerprinting process for Nursing covers new licensing and renewals The form for Fingerprint Processing can be found within your Application (Instructions for Completion and Submission of Fingerprints or Fingerprint Voucher) for new and renewing licensing.
We can also provide the form for you and help you to fill it out.

Nevada State Contractors Board
The current form (Fingerprint Verification Form) required for fingerprinting servicesis emailed to the applicant after the board receives their application. This form CANNOT BE PROVIDED by B&D Fingerprinting, as applicants are now required to start their process with the Nevada State Contractor's Board PRIOR to receiving Fingerprinting Services.

Nevada Real Estate Division
The fingerprinting process for Real Estate covers new licensing and renewals. B&D Fingerprinting can process your fingerprints and submit them electronically to the Nevada State Department of Public Safety where the results will be sent back to the proper division. State of Nevada Private Investigator's Licensing Board
The fingerprinting process for the Private Investigator's Licensing Board covers new licensing and renewals. As of October 6, 2014, the PILB has changed their application and fingerprint requirement process. Individuals must now go to the Private Investigator's Licensing Board directly or on their website, and pay the PILB Fee for processing prior to obtaining fingerprints. Once payment has been confirmed, individuals will receive a stamped electronic submission form. All applicants must retrieve this form (Applicant Electronic Submission Form) from the PILB prior to obtaining fingerprints. Once obtained, B&D Fingerprinting can process your fingerprints and submit them electronically to the Nevada State Department of Public Safety where the results will be sent back to the proper division. Please make certain all fields are filled correctly; if you have any questions or concerns over the paperwork, our fingerprint technicians can help you through the process. The Private Investigator's Licensing Board also requires a photo, which we can also provide for you.
