Nevada Medical Marijuana Program (Patient Cards)

Process

The fingerprinting process for the Medical Marijuana Program for the state of Nevada covers new applicants and renewals. This process is specifically for patients requesting access to Medical Marijuana. For Agents and Establisments, please click here. Applicants must first contact the Medical Marijuana Registry in order to obtain an application. An Application Request Form can be fown here. Applications require a copy of the front and back of the applicant's driver's license or state-issued ID. Please visit the Medical Marijuana Registry's website for the most up-to-date information on the process of obtaining their application.

B&D Fingerprinting Services LLC can process your fingerprints onto the FBI standard FD-258 Fingerprint Cards to be sent in with your application to the Medical Marijuana Registry. If you have any questions on the process or program, contact the Nevada State Health Division for more information on the process.

Forms

No forms are currently required to be brought in for fingerprinting process. The Medical Marijuana Registry will, however, require their application to be filled in completion with fingerprints. Fingerprints will be processed onto one FBI standard FD-258 Fingerprint Card. Fingerprints will be captured via Livescan Technology and printed onto the card.

The Next Step

B&D Fingerprinting Services LLC will provide you with the standard FD-258 Fingerprint Card. This card is to be placed with your paperwork and to be taken to the Medical Marijuana Registry. B&D Fingerprinting Services LLC has a Notary Service with specific hours at our Executive Office Location. They will notarize your application on-site. With a completed application, applicants should then ship all required paperwork and and Fingerprint Card directly to the Medical Marijuana Registry. The address can be found below.
